First Visit Checklist
In order to make your appointment as productive and as informative as possible, please be sure to bring along the following information:
- Referral (if necessary)
- Photo identification
- Insurance information (primary and secondary)
- Co-pay for a specialist visit (visa, mastercard, cash and checks accepted)
- Pharmacy telephone number and fax number
- List of current medications (prescription, over-the-counter, or herbal preparations) with dosage and frequency
- Complete Patient Information Form (available soon).
- Reports and actual images (CD or film) of x-rays, CT scans, nuclear medicine studies, or MRI scans
- Reports of laboratory studies
- Summaries or medical records from referring physician
